The full formal description of Spirit Forge is below.  I was recently asked to confirm our understanding with national and a national rules representative confirmed it for us.

If you get more than 1 Spirit Forge cast on your character in a one year period you get a Black Marble and a note on your character card with the date and the number of permanent deaths that may NEVER be removed from the character card / database. Even if the database may currently allow the buy back, the note on the character card should be respected and the player not allowed to buy back the black.

National confirmed this with a simple "They should never be removed." in reference to deaths received in this manner.

This formal magic tears the target spirit apart and forges it anew. The spirit forgets its previous
profession and all skills but may choose a new profession and relearn skills immediately.
Spirit Forge is instantaneous and irrevocable.
The player may immediately reenter their character with identical experience and any changes
to the class and skills in the character database. The formal magic marshal must enter the date of
the Spirit Forge in the Formal Magic logbook for entry into the character database at the earliest
convenience. Deaths taken from this formal magic may NEVER be bought back with goblin stamps
in any chapter..

The process of Spirit Forge (from any source) may weaken the spirit. If the process of Spirit Forge
is performed upon the same spirit more than once within a year it will be weakened each time by a
number of deaths equal to the number of times it has spirit forged other than the first.
For this reason
the date of each Spirit Forge effect must be listed on the playerís character card.
For example, this is listed on Lumís character card
October 12 1999, Spirit Forge (first ever)
October 13 1999, Spirit Forge (one death)
October 11 2000, Spirit Forge (two deaths)
October 13 2000, Spirit Forge (one death)
October 13 2001, Spirit Forge (no death)

It does go on to explain that the death "wears off" after the period of a year

No it does not. You are misreading the example with the deaths listed.
 The quote with 1 word change... "a year" for "the last 365 days"

If the process of Spirit Forge is performed upon the same spirit more than once within "the last 365 days" it will be weakened each time by a number of deaths equal to the number of times it has spirit forged other than the first.

For example, this is listed on Lumís character card
October 12 1999, Spirit Forge (first ever)   
October 13 1999, Spirit Forge (one death)        *second death in the last 365 days = 1 death / black marble added
October 11 2000, Spirit Forge (two deaths)      *third death in the last 365 days = 2 deaths / black marbles added
October 13 2000, Spirit Forge (one death)       *second death in the last 365 days = 1 death / black marble added
October 13 2001, Spirit Forge (no death)          *first death in the last 365 days = 0 death  black marbles added

The are not "expiring" the example is just showing that it is the last 365 days, not the last calendar year.  Specifically the October 13, 2000 date is just after the October 12 1999, and October 13 1999 castings is no longer count as being in the same 365 days window so the October 13 2000 is only the second casting in the last year (with October 11 2000) and only ADDS 1 death.

The character in this example has 4 deaths on his card that can never be removed.
